Notice of Privacy Practices

Effective date: October 19, 2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Feidy Chiropractic Clinic is required by law to keep your protected health information (“PHI”) private, to give you this notice of our legal duties and privacy practices, and to follow the notice that is currently in effect. PHI is information that identifies you and relates to your past, present, or future health, the care you receive, or payment for that care.

How we may use and disclose your health information

For treatment

We may use and share your PHI to provide and coordinate your care. For example, your chiropractor may review your intake form, X-rays, and progress notes, or may share information with your primary care doctor, a specialist, or an imaging center you are referred to.

For payment

We may use and share your PHI to bill for and collect payment for your care. For example, we may send your diagnosis and treatment information to your health insurer, an auto insurer, or a workers’ compensation carrier.

For health care operations

We may use and share your PHI to run our practice. Examples include quality reviews, staff training, licensing, accreditation, and business planning.

Business associates

Some services are provided for us by outside companies, called business associates. Examples include our online scheduling and intake-form provider, billing services, and IT vendors. Business associates must protect your information under a written agreement with us.

Appointment reminders and health-related services

We may contact you by phone, voicemail, text message, email, or mail to remind you of appointments. We may also tell you about treatment options or health-related services that may interest you.

Family members and others involved in your care

Unless you object, we may share information with a family member, friend, or other person you identify who is involved in your care or helps pay for it. In an emergency, or if you cannot agree or object, we will use our professional judgment to decide whether sharing is in your best interest.

Other uses and disclosures allowed or required by law

We may use or share your PHI without your written permission in the following situations, within the limits set by law:

  • When the law requires it. This includes federal, state, or local law.
  • Public health activities. For example, preventing or controlling disease, reporting adverse reactions to products, or reporting child abuse or neglect.
  • Victims of abuse, neglect, or domestic violence. We may report to government authorities as the law requires or allows.
  • Health oversight activities. For example, audits, inspections, investigations, and licensure by agencies such as the Georgia Board of Chiropractic Examiners.
  • Lawsuits and legal proceedings. In response to a court or administrative order, or to a subpoena or other lawful request.
  • Law enforcement. For specific purposes the law allows, such as responding to a court order or helping to identify a suspect or missing person.
  • To prevent a serious threat to health or safety. When necessary to prevent a serious and imminent threat to you, another person, or the public.
  • Coroners, medical examiners, and funeral directors. As needed for them to carry out their duties.
  • Organ and tissue donation. To organizations that handle organ, eye, or tissue procurement or transplantation.
  • Research. Only when an institutional review or privacy board has approved it and privacy protections are in place.
  • Specialized government functions. For example, military and veterans’ activities, national security, and protective services for the President.
  • Workers’ compensation. As state law on workers’ compensation and similar programs allows.
  • Inmates. To a correctional institution or law enforcement official that has custody of you.
  • U.S. Department of Health and Human Services. When HHS asks for information to check that we comply with privacy laws.

Georgia law gives some kinds of information extra protection, such as information about HIV/AIDS, mental health, and substance use. Where state law is stricter than federal law, we follow state law.

Uses that require your written authorization

We will not do the following without your written permission:

  • Use or share psychotherapy notes, unless the law allows it
  • Use or share your PHI for marketing
  • Sell your PHI
  • Use or share your PHI for any purpose not described in this notice

You may cancel a written authorization at any time by writing to us. Cancelling stops future uses and disclosures. It does not undo anything we already did with your permission.

We do not use PHI for fundraising.

Your rights

  • To see and get a copy of your records. You may ask to see or get a copy of your medical and billing records, including an electronic copy. We will usually respond within 30 days and may charge a reasonable, cost-based fee.
  • To ask us to correct your records. If you think information in your record is wrong or incomplete, you may ask us in writing to correct it. We may say no, but we will explain why in writing within 60 days.
  • To ask for confidential communications. You may ask us to contact you in a specific way, such as only on your cell phone, or at a different address. We will agree to all reasonable requests.
  • To ask us to limit what we use or share. You may ask us not to use or share certain information for treatment, payment, or operations. We do not have to agree, except in one case: if you pay for a service in full yourself, you may ask us not to share information about that service with your health plan, and we will agree unless the law requires us to share it.
  • To get a list of disclosures. You may ask for a list (an “accounting”) of the times we shared your PHI in the six years before your request, along with who we shared it with and why. The list will not include disclosures for treatment, payment, or operations, or certain other disclosures. We will provide one list a year for free.
  • To get a paper copy of this notice. You may ask for a paper copy at any time, even if you agreed to receive it electronically.
  • To choose someone to act for you. If someone is your medical power of attorney or legal guardian, that person can use your rights and make choices about your health information.
  • To be notified of a breach. We will notify you if a breach occurs that may have compromised the privacy or security of your PHI.

To use any of these rights, contact us using the information at the end of this notice.

Changes to this notice

We may change this notice. The changes will apply to all information we have about you. The new notice will be available on request, in our office, and on this website.
